Discharge Information
When your doctor believes you no longer require hospitalization, he or she will write an order for discharge. In most cases, you will be prepared for discharge on our STAR unit, where you will be given information about any precautions you need to take, where to call to make follow-up appointments and what prescription medications to take and when.
Your STAR nurse will ask you for a phone number so that he or she can check on you within 72 hours after discharge to ensure you are recovering as expected.
Patients also have access to a case manager to assist them with discharge planning. You may notify your nurse to see a case manager or call the case management department at 244-4133 from 8:30 a.m. to 5 p.m. After-hours requests will be responded to the following day.
A random selection of our patients will be contacted by phone to participate in a patient satisfaction survey. If you're feeling up to it, please take the time to give the surveyor your honest feedback.
Make arrangements with a family member or friend to drive you home from the hospital or to help you arrange transportation. For assistance from the Community Transportation System, call (904) 393-4200.
