Patients need to be seen by their primary care physician to determine the need for a sleep study. Once the completed order is received, a representative from the Sleep Disorders Center will contact the patient to schedule an appointment.

We will call to confirm the appointment. If a confirmation call is not received by the patient for any reason, the patient will need to call the office no later than 3 p.m. the day of the appointment to confirm. If confirmation is not received, the appointment will be cancelled and the patient will need to make another appointment.

Payment for Services

We will be happy to file for insurance payment after any deductible or co-insurance requirements have been met. If insurance cannot be verified or if the correct referral number is not provided, the patient may be responsible for payment. Payment is required at the time of service. We accept cash, personal check, MasterCard, Visa and payroll deduction for Shands Jacksonville and University of Florida employees.
