Medical Services and Treatments
Services provided by Jacksonville Health and Transition Services are covered by Medicaid, Children's Medical Services and most other health insurance plans and include:
- Comprehensive intake, evaluation and healthcare planning.
- Assistance securing health insurance coverage and other funding resources.
- Referrals to a reliable network of adult medical and surgical specialists, as well as community providers in education, vocational transition services, living arrangements, social/recreational and employment.
- Training that supports the shift of responsibilities from parents to young adults, prepares them to be active members of the healthcare team and promotes the development of self-care skills and independence.
- Coordination with other agencies that provide transition services to young people with special healthcare needs, including public schools, Division of Vocational Rehabilitation, Florida Community College at Jacksonville, ARC of Jacksonville and Hope Haven.
