Healthcare Services Pediatrics Transitional Services

Teens and young adults with disabilities and special healthcare needs must deal with many complex issues during the transition from childhood to adulthood. Learning to take charge of their health is important for promoting success in employment, lifelong learning and independent community living.

Helping them make the move from pediatric to adult-oriented care, Jacksonville Health and Transition Services at Shands Jacksonville works with teens and young adults, ages 14 to 25, with chronic medical or developmental conditions living in Duval, Nassau, Baker, Clay and St. Johns counties.

The multidisciplinary staff includes a University of Florida pediatrician, adult internal medicine specialist, nurse care coordinators and a transition specialist who provide primary medical care to address the young person's immediate needs. The team also works with youth to help them gain the knowledge and skills they need to achieve their long-term goals.
