To refer a patient to the UF Department of Neurosurgery at Shands Jacksonville, physicians can call (904) 244-3950 or fax a completed referral form to (904) 244-9648.

The referring physician is notified of the patient's appointment within 24 hours. Patients will be seen within seven to 14 days. Referring physicians are continually updated on their patient's progress and receive dictated notes from attending physicians.
