Community Outreach Programs
Specialty Care for Children in Rehabilitation Services
The Division of Community Pediatrics and Primary Care provides a staff pediatrician as a member of the comprehensive inpatient rehabilitation team serving children with severe physical and cognitive injuries at Brooks Rehabilitation Hospital. During regular bedside visits, the pediatrician oversees the medical needs of the child and offers support for the family while the child receives intensive rehabilitation therapy.
Cleft Lip and Palate Team
The cleft lip and palate team provides comprehensive care, surgery, therapy and management of children from birth through adolescence to ensure children have the best possible outcomes. The team consists of pediatricians, plastic surgeons, dentists, oral surgeons and orthodontists, otolaryngologists, speech and hearing professionals, a geneticist, social worker and nurse coordinator. Upon referral from the nursery or physician, the nurse provides parental support and education on feeding and coordinates the various medical services a child may require. Once in the program, children receive a comprehensive evaluation from our interdisciplinary team and a plan is developed and implemented in consultation with the family.
Community PedsCare
Community PedsCare provides comprehensive and compassionate palliative care to children with potentially life-limiting and chronic conditions, as well as end-of-life care to children and their families. The program is a part of Community Hospice of Northeast Florida and works in collaboration with physicians and other professionals and organizations throughout the region to ensure children and families receive care designed to relieve suffering, provide comfort and improve the quality of their lives. Services include pain and symptom management; medical consultation; mental, psychosocial and spiritual support and counseling; family respite; assistance with financial issues; case management; and bereavement and grief support. Special attention is paid to the needs of siblings.

Medical Home for Homeless Children Project
MHHCP is a system of care provided to homeless families living in Jacksonville's shelters. Nurse care coordinators visit the shelters and bring families into a medical home at the Kids 'N Care Health Center. They provide case management, transportation and health education to these families.
JaxHATS
The transition from pediatric to adult-oriented care can be very complicated for those with disabilities and chronic health conditions. The Jacksonville Health and Transition Services (JaxHATS) clinic was created to help make this transition easier. JaxHATS serves teens and young adults, ages 14-25, with chronic medical or developmental problems. The JaxHATS multidisciplinary staff includes a pediatrician, adult internal medicine specialist, nurse care coordinators and a transition specialist who provide primary medical care to address the young person's immediate medical needs. The JaxHATS team also works with youth over time to help them gain the knowledge and skills they need to achieve their long-term life goals. Services are open to residents of Duval, Nassau, Baker, Clay and St. Johns counties and are covered by Medicaid, Children's Medical Services and most other health insurance plans. More information...
Kids 'N Care
Kids 'N Care is a unique program in Florida that provides comprehensive medical, mental, developmental and behavioral assessments and care to children in the foster care system. The program works in collaboration with Family Support Services, Department of Children and Families and multiple community organizations to ensure these at-risk children and families have access to the care they need.
