Benefit Eligibility Explanation

Medical Insurance

Full and part-time employees working 48 or more hours per pay period.

Benefits begin the first day of the month following 30 days of employment.

Includes a Shands Jacksonville (SJ) option and a Humana National POS option.

Employee premiums range from $21.23 to $161.39 depending on option and type of coverage.

Prescriptions range from $12 to 70 percent of medication cost.

Dental Insurance

Full and part-time employees working 48 or more hours per pay period.

Benefits begin the first day of the month following 30 days of employment

Includes PPO and DMO plans with 100 percent preventative coverage of $5 office visit co-pay. No claim forms required unless you choose an out-of-network provider.

Vision Care Plan

Full and part-time employees working 48 or more hours per pay period.

Benefits begin the first day of the month following 30 days of employment

Plan pays 75 percent - up to $50 per person per year - for eye exams and 75 percent of covered expenses for eyeglass frames, lenses (one set per plan year) and contact lenses after $25 deductible

Each plan participant can receive up to $250 in benefits each calendar year.

Basic Group Life

Full and part-time employees working 48 or more hours per pay period.

Benefits begin the first day of the month following 30 days of employment.

SJ provides employees life insurance valued at one times their annual salary. Employees can also purchase an additional one, two or three times annual salary.

Long Term Disability

Full time employees who work 32 hours per week after one year of service.

Paid to employee in the event that of a disability, on or off the job, after 180 days.

401(k) Retirement Plan

Full and part-time employees who have completed 12 months and 1,000 hours of employment.

SJ automatically gives eligible employees 3 percent of salary towards the investment opportunities elected by you, regardless of whether the employee contributes. In addition, you can defer up to 6 percent of your salary and SJ will match dollar for dollar on the fourth, fifth and sixth percentage you contribute.

Flexible Spending Accounts (FSA) for Healthcare and Dependent Care

Full and part-time employees working 48 or more hours per pay period.

Allows employee to set aside pre-tax dollars to pay for eligible medical expenses up to $2,500 and dependent care expenses up to $5,000 per year.

Full and part-time employees on the first day of employment

All of these are available for emplyees to purchase at the SJ group rate. Carrier information provided at new employee orientation. Benefits premiums deducted bi-weekly and paid by payroll deduction.

Tuition Reimbursement

Full and part-time employees working 48 or more hours per pay period

Assistance available to employees enrolled in courses that will benefit them in their current or prospective position at SJ.

Grade C or better = 100 percent reimbursement

Textbooks are reimbursed at 50 percent

Max benefit per fiscal year (July 1-June 30):

  • Certificate program/exam/GED: $800
  • Bachelor's and associate degree programs: $1800
  • Advanced degree programs: $2750

Employee Assistance Program (EAP)

Full and part-time employees on the first day of employment

Free and confidential service which provides to balance work/life issues.

Home Ownership Program

After one year of continuous service

Up to $5,000 "soft second" loan at closing for home purchased and revitalized in Historic Springfield. Loan fully forgiven after five years of service.

Paid Personal Leave

Full and part-time employees on the first day of employment

Used for vacation, illness, absences for personal reasons and holidays. Employees begin accruing up to seven hours per pay period on first day of employment.

Bereavement Leave

Full and part-time employees at the completion of probationary period

Provides five calendar days off, of which the first three are paid as bereavement and the remaining charged as PL.
