Residency Programs
Application Process
All of the following are required for your application to a PGY-1 or PGY-2 pharmacy residency at Shands Jacksonville to be complete:
- Residency application
- Letter of intent (address to program director, typically about 1 page in length)
- Curriculum vitae
- Three (3) letters of reference from professional sources (using the attached form, often accompanied by a letter also)
- Official college transcripts (for pharmacy coursework, should be sealed)
